Missouri Quality Initiative for Nursing Homes

In November 2012, Missouri Health Connection (MHC) was selected by The University of Missouri Sinclair School of Nursing to serve as its technical partner on a nearly $15 million grant from the U.S. Department of Health and Human Services Centers for Medicare and Medicaid Services (CMS). The grant award’s goal is to reduce avoidable re-hospitalizations of nursing home residents, and is one of seven awards in the country. This important project aims to improve patient care and lower health care costs and was renewed by CMS in 2016.

Through the Missouri Quality Initiative for Nursing Homes, MHC, the MU Sinclair School of Nursing, CMS, and Primaris work together to improve care for residents at 16 nursing facilities in the greater St. Louis area by increasing communication and coordination.
